If preferred, you can click here to download and print the intake paperwork from home and bring it to our office already filled out.
Once the paperwork is submitted, we will schedule you for an initial assessment that will assist us in better serving you. The assessment can often be scheduled within just a few days of receiving the paperwork. Typically, once the assessment is completed, you can begin receiving the requested service.
